We would appreciate any advice. My fridge is at the top of the van, so I don’t have to bend down too much.😁. We have bought a 12 V computer fan should I suck the cold air in from under the van through a hole in the fridge and vent it through the hole in the side of the van at the back of the fridge or should the air be blown out through the top vent using the fan?. we are struggling to keep the temperature controlled when it’s warm outside
 
I'm struggling to work out how your fridge is vented, if it's a 3way standard vents are at top and bottom of fridge on the outside wall. this allows venting of the fridge and access for maintenance. Here's a photo of a van with high level fridge and same vents as ours

The usual method is to put a fan (or fans) behind the top vent blowing warm air out. If you are restricted by lack of space due to your fridge arrangement you could put the fan behind the lower vent blowing air in from outside.
