It is worth bearing in mind that repatriation is not a choice of the policy holder but is decided upon by the insurer - and it is very expensive, so they are likely to look first at other alternatives. If you are travelling to, say, Chad in north Africa and suffer a heart attack then they may welll decide that repatriation is the only answer but if you are in, say, Spain then it is highly likely that they will decide that the treatment you can get locally is at least the equal of that you would get at home - and a cheaper option for the insurers.

My husband was repatriated from Spain to the UK with medical nurse doctor,

A few months ago there was a couple and the wife had to be brought home from Spain but had no insurance so her family and friends did fairs, fetes. car boots, sponsors , to generate enough money to bring there love one home and on the local news not long ago a man was in the same predicament in Portugal,

Hope your husband is ok. I did not mean to imply that you would not get repatriated from Spain but that the insurance company is more likely to decide that suitable treatment is available there than is, say, a third world country - and having sampled the hospital treatment in Spain we are extremely happy with it. That is why we are among those who choose not to take out additional insurance in the EU but always do it when travelling further afield. It is, of course, a matter of personal choice but a lot of people don't realise that if you take out insurance that covers repatriation it does nor mean that you can choose to be repatriated whatever.
